    A wrongful death is a fatality caused by the negligence, wrongful act, neglect, omission, or default of someone else. According to a study by the Institute of Medicine (IOM), one in five medical mistakes are serious or fatal. As such, an estimated 98,000 Americans die each year due to medical errors. This includes 7,000 deaths caused by medication mistakes.

    Additionally, The CDC reported there were 19,656 slip-and-fall fatalities in 2005. Factors in these deaths include obstructed slippery walkways, poorly lit walking areas, stairways without railing, lack of safety signs, and use of unapproved ladders and other equipment.

    According to the U.S. Department of Labor, in 2006 there were 5,840 fatalities resulting from workplace mishaps. Some of the causes of these workplace deaths were traffic incidents (2,459 deaths), assault and violent acts (788 deaths), contact with objects or equipment (993 deaths), Falls (827 deaths), and exposure to harmful substances (547 deaths).
